What this page covers
This notice explains what happens to the information you enter into the suitability check form on this website. It applies to that form. Once you become a client, the practice's own client privacy and confidentiality terms apply to your records, and those will be explained to you before your first session.
What we collect
Through the form we collect your name, email address, phone number, your contact preferences, which therapist you would prefer, and your answers about what you are experiencing and your therapy history. Some of that is health information, which Australian privacy law treats as sensitive information. That is why the form asks for your consent before you send it.
Why we collect it
So that a therapist can read your answers before anyone calls you, form a view on whether EMDR is the right next step, and match you with a therapist who has availability and works the way you need. It is also used to contact you about your enquiry.
Who sees it
Kellie Sheldon's practice team, which is the three therapists and the practice administrator. It is not sold, rented or shared for marketing. It is held in the practice's enquiry system and is not published anywhere.
You do not have to tell us everything
Every question about what you are experiencing can be answered with a general option, and one of them is 'I would rather not say yet'. The free text box at the end is optional. You will not be treated differently for leaving things out, and nothing you write here is required before you can book.
If you change your mind
You can ask us to delete your enquiry at any time by emailing admin@kelliesheldon.com.au, and it will be removed. You can also ask to see what was recorded, or ask for it to be corrected.
If something goes wrong
If you have a concern about how your information has been handled, contact the practice first on 0481 450 124 or admin@kelliesheldon.com.au. If it is not resolved, you can raise it with the Office of the Australian Information Commissioner at oaic.gov.au.
